Media Release - By submitting this form, I authorize Inspired Paws of Cookeville to use the following personal information: (1) My picture and or my dogs- including photographic, motion picture and electronic (video) images. (2) My voice- including sound and audio/video recordings. I hereby grant Inspired Paws of Cookeville the right to use, publish and reproduce, for all purposes, my name, pictures of me in film or electronic (video) form, sound and video recordings of my voice, and printed and electronic copy of the information described in sections (1) and (2) above in any and all media including, without limitation, cable and broadcast television and the internet, and for exhibition, distribution, promotion, advertising, press conferences and in brochures and other printed media. This permission extends to all languages, media, formats and markets now known or hereafter devised.(Required)

Inspired Paws of Cookeville LLC weighs each pet on drop off. I understand that my daily rate may change upon drop off if my pets weight is different than quoted.(Required)

Drop offs and pick ups are by appointments only. I understand if I pick up my pet a day(s) earlier than booked, I will pay for the dates that were booked if my dates cannot be filled by another client.(Required)

I understand if I pick up not within the available hours I will be required to pay an extra fee. Our hours as of May 1, 2025 are Sunday 3:00 pm to 7:00 pm, Monday - Saturday 10:00 am to 7:00 pm.(Required)

I certify that the pet brought to Inspired Paws of Cookeville LLC belongs to me and/or I am authorized to make all arrangements for the owner and Inspired Paws of Cookeville LLC is authorized to make all decisions for the pet while in its care.*(Required)

I acknowledge that the management of Inspired Paws of Cookeville LLC requires my pet(s) to be current on vaccinations and free of external and internal parasites: DOGS – Rabies, DAPPV, Parvo. Bordetella (Kennel Cough) And recommends: Canine Influenza**(Required)

I understand that communicable diseases can and do occur when dogs are allowed to interact and/or are housed in the same facility. I understand that although Inspired Paws of Cookeville LLC practices exceptional sanitation, the risk of exposure still exists, regardless of vaccine history, and accept responsibility for my dog’s exposure.(Required)

I understand that dogs who interact together may leave marks on one another, via play or unpredicted aggression. Those marks may be left from tooth, nail or other. Knowing this, I choose:*

Emergency Medical Authorization - In the event of illness or injury, if I can not be reached, I authorize Inspired Paws of Cookeville LLC to use any licensed veterinarian or my vet and for my pet to be treated as advised by that veterinarian. NOTICE TO ANY AND ALL VETERINARIANS: I hereby place Inspired Paws of Cookeville LLC as the representative on my behalf of my pet. I will be responsible for any and all payments regarding the treatment of my pets. ***Please note, pets can not be left in the care of Inspired Paws of Cookeville without owners giving permission for emergency treatment. ****

In the event of a minor medical event (torn toe nail, loose stools, new experience stress, etc), does Inspired Paws of Cookeville LLC have permission to administer first aid, administer supplements, or medication as prescribed by a veterinarian?(Required)

I acknowledge that Inspired Paws of Cookeville LLC will NOT be held liable for injury, illness, theft, fire or accident to any animal in its care. I agree to be responsible for any damage done by my pet to persons or property while in the care of Inspired Paws of Cookevile. If I do not retrieve my pet by the agreed upon date and if no further arrangements are made for my animal’s care, I acknowledge that Inspired Paws of Cookeville LLC has the right to dispose of my animal 10 days after notification according to the law. By printing my full name, I state this is binding on all transactions, current and future. I have read and understand the agreement.(Required)

Brush-Up Training

Dogs with previous training will have the option to add brush-up training to their boarding!

Hang Out in the Office

$10 with this add on your pet can get extra hang out time and hang out with the staff while we do our daily routine in the office! We can allow office time once a day for about 30 minutes!

Adventure Creek Walks

$17 for a fun walk which includes a hike with hills and a creek adventure walk.

Sniffari Walk

$12 for a fun walk which includes a walk down the road from the kennel which is a mile.

Treadmill Walks

Treadmill walks are offered by the minute at the price of $1 per minute

Frozen Enrichment Feeders

$4/day extra, no charge if clients bring their own stuffed enrichment toys. This includes toys stuffed with yogurt, blueberries, bananas, peanut butter, canned food, chicken pot pie recipe, and more! Client has a choice of ingredients.

Dry Enrichment Feeders

$2/day extra, no charge if clients bring their own enrichment toys. We supply Komg Wobblers, Snoops, Pet Safe Magic Mushroom, Slow Feeders, Squirrel Dudes, and so much more!

Refreshing Go Home Groom

Ranging from $25 to $45 depending on weight of pet and thickness of coat

Kennel Food

For $2 extra a day we can feed your pets one of our formulas we keep on hand. Formulas: Proplan Puppy Chicken and Rice, Proplan Salmon and Rice, Wholesome High Energy 26/18, Wholesome Chicken Meal and Rice.
